Patient Prescriptions

• Download the order form and fill it in.
• Submit CLEAR images of ALL copies of the prescription AND the order form via Fax (03) 9646 0991 or email orders@creativepharmacy.com.au 
• If you wish to have your order posted to you, mail all prescriptions and the order form to 86 Bay Street Port Melbourne 3207. 
• If you are picking up your order, you must bring in the paperwork. We will notify you by SMS or phone when your order is ready for collection.
• We will file your prescription at our pharmacy so that you can easily re-order, unless you specify otherwise.

Refrigerated Medication Policy

• To ensure safety and efficacy of your medication we recommend picking up medications that require refrigeration, particularly on warm days.
• If you are unable to pick up your medication, we will endevour to express post your products with ice at your own risk. It is then your responsibility to ensure that you retrieve your parcel immediately. Please provide our friendly staff with your email so that you may track your order.
